Olivet Nazarene University's graduate handbook asks for positive titers, a negative TB result every year, a Tdap within 10 years, a yearly flu vaccine and an in-person American Heart Association BLS card, with your physical recorded on Olivet's Health and Immunization Form. The physical and Olivet's Health and Immunization Form

Start with the form. The new-curriculum handbook asks for a physical exam by a nurse practitioner, a physician (MD or DO) or a physician assistant, recorded on Olivet's own Health and Immunization Form. Take the form with you to the appointment so your provider completes it at the visit.

Once the physical is done, the rest of the health items fall into three groups: titers that prove immunity, vaccines with a date limit, and a test you repeat every year. Each file you upload should identify you by name and demographic details, state the finding and carry a date, which is what Olivet looks for on every compliance record.

The graduate list, with the non-health items and their deadlines, is on the clinical compliance page. Your records go into the system your handbook names, Verified Credentials QualifiedFirst for the new curriculum or CastleBranch for the current one.

Titers for MMR, varicella and hepatitis B

Olivet asks graduate students for positive titers, not only vaccination records, for measles, mumps and rubella (MMR), varicella and hepatitis B. A titer is a blood test that measures immunity, so the document you submit is a lab report showing the result.

When a titer comes back negative, the handbook's answer is a booster followed by a repeat titer. That adds a shot and a second blood draw before the item is complete, which is why titers are best drawn early in the program rather than in the weeks before your first clinical.

Declining the hepatitis B vaccine requires a medical statement from a provider. If you plan to decline, arrange that statement at the same time as your titers.

Traditional BSN students follow a different rule: two MMR and two varicella doses or titers, and the full three-dose hepatitis B series, as the BSN clinicals page explains.

TB: a negative result every year

The graduate handbook accepts four ways to show you are TB negative. The result has to be negative, and it has to be renewed every year.

For an FNP sequence of four or six clinical courses, the TB anniversary is one of the dates that can fall in the middle of a course, so note it on the day you upload. New RN-BSN students have a shorter list, a 2-step TB or QuantiFERON within 12 months, and Traditional BSN students begin with a 2-step TB and repeat it every year. Whichever test you choose, the date on the result starts the next yearly clock. Testing in a quiet month between courses, rather than the week before a clinical opens, sets a renewal date that is easier to meet the following year.

Tdap and the yearly flu vaccine

Tdap must fall within the last 10 years. The RN-BSN handbook for new students adds an alternative: a lifetime Tdap plus a Td booster within 10 years also qualifies.

The flu vaccine is yearly. Declining it, like declining hepatitis B, needs a provider's medical statement under the graduate handbook. Traditional BSN students use a flu exemption form instead, due no later than the semester before their first rotation, and their handbook notes that facilities decide whether to accept it.

Graduate students should keep the same possibility in mind, because Olivet's handbook says sites may require more than Olivet does. A clinic can set its own vaccine rule, so ask about it before you rely on a declination at that site.

BLS: American Heart Association, in person, valid to the end

The graduate handbook asks for an American Heart Association BLS card from an in-person course, and online courses are not accepted. The card has to be valid through the whole practicum, so its expiry date matters as much as the card itself.

If your card will expire during an 8-week clinical course, renew before the course begins. If you hold BLS through your employer, confirm before uploading that the card is AHA-issued and that the course was taken in person.

RN-BSN students need AHA BLS for Healthcare Providers. Traditional BSN students take BLS in person, and the nursing administrative assistant collects their cards.

An online-only BLS course does not meet Olivet's graduate requirement. The class must be in person and the card must be American Heart Association.

A renewal calendar for your clinical sequence

With everything uploaded, the job becomes keeping it current. Olivet's standard is complete before the first clinical or practicum day and current throughout, so keep one calendar with both your course dates and your expiry dates.

Does Olivet accept vaccination records instead of titers?

Not for graduate students. The new-curriculum handbook asks for positive titers for MMR, varicella and hepatitis B, and a negative titer means a booster and a repeat titer. Can I decline the flu shot for Olivet clinicals?

Is a chest x-ray accepted instead of a TB test?

Does an online BLS renewal count for Olivet?

Not for graduate clinicals. The handbook requires an American Heart Association BLS card from an in-person course and states that online courses are not accepted. The card also has to stay valid through your whole practicum, so time the renewal before a course starts rather than in the middle of one.

Who can complete my Health and Immunization Form?
